Overview

Holy Name Medical Center is dedicated to providing its patients with as much information as possible regarding potential health care costs.

Please read each of the following 3 sections carefully. Each one contains important information you need to be aware of in order to make the proper decisions concerning your course of treatment.

Plans we participate in

To start this process, you should see if your insurance plan is accepted at Holy Name. Click 'View The List Of Insurance Plans' below to see the list of insurance plans Holy Name participates in. If you do not see your plan listed, we strongly recommend that you contact us or your health insurance company to confirm whether your plan is considered in-network at Holy Name or out-of-network.

It is also important that you call your insurance company prior to receiving hospital care. Each plan is different, and some provide different levels of coverage that could make a difference in your out-of-pocket costs. It is always best to contact your insurance company to get more information.

In addition, the physicians who provide care within our hospital might not participate in the same insurance plans as the hospital. You should check with the physician who is arranging your hospital care to see which insurance plans the physician participates in. You should also know that these health care professional charges are not included in what Holy Name charges. They will bill you separately.

Plans we participate in

Click below to see a list of the physicians that are employed by Holy Name. You may still receive a separate bill for their services if they process their own billing. These physicians' charges are not included in what Holy Name charges.

Physicians We Contract With

Click below to see a list of the physicians that are contracted with Holy Name Medical Center. They are not hospital employees. Instead, they have a contract with us to provide services within the hospital. You should also know that these physicians' charges are never included in what Holy Name charges. They will bill you separately.

The list also includes nurse practitioners (NP's) and physician assistants (PA's) who are contracted to provide services to Holy Name and its patients.

No Surprises Act

Centers for Medicare & Medicaid Services (CMS) has instituted regulations to protect patients from surprise medical bills. More details on the No Surprises Act can be found at cms.gov/nosurprises. The No Surprises Act protects patients with insurance through an employer, individual health plan or Health Insurance Marketplace from balance billing for designated services. Details of these balance billing protections can be found in this CMS Disclosure (PDF). Patients with no insurance will be provided a good-faith estimate for pre-scheduled appointments.
