What is an Advance Directive and why do I need one?
An Advance Directive is a 2-part document that explains your wishes for your medical care if you are unable to communicate, and also designates someone (a healthcare proxy) to make decisions for you. It helps ensure that your loved ones know what you want if you are in a life-threatening medical situation, which can help prevent family conflicts.
What if I'm undecided about what I want in some of the circumstances outlined in the Advance Directive?
If you are undecided, there is a choice that indicates your healthcare proxy should make the decision.
Do I need a lawyer to make this a legal document?
You do not need a lawyer, but you do need two witnesses to sign the document once it's completed. There is a place for them to sign on the Summary page, which pops up after you finish answering the questions.
Where should I keep my Advance Directive?
You can keep this digital document on any of your mobile devices, or you can print and store it in a convenient place. Be sure to give copies to your healthcare proxy and your loved ones.
What if I have other medical documents I want to keep with my Advance Directive?
There is a place for you to upload other documents as well as an opportunity to upload a personal video you may want to create that speaks directly to your loved ones.
